Carers often worry about what will happen to their family member if they become ill suddenly, or die.  Complete our Planning Tools to ensure peace of mind for you, and uninterrupted care for the person you support if the worst does happen.

Update these Tools regularly if the information changes.  We'll be adding to this section of our site during 2010, and print copies of the Tools will be available later in the year.

If you are a 24 hour carer, ensure that emergency medical personnel are aware that your family member needs help if you are injured, are suddenly incapacitated, or die. Print several copies of this Emergency Carer Identity Card.
